All complaints and feedback are confidential and are treated in accordance with the TAG 5 confidentiality policy and complaints policy. Please contact us to request a copy of these policies.

 

Complaints and Feedback can be submitted in the following ways:

  • Filling out the online form provided below
  • ​Downloading, printing and filling out the Complaints and Feedback PDF and emailing it to enquiries@tag5.com.au
  • Phoning Head Office: (07) 3036 2852

If your complaint is unresolved by TAG 5, contact can be made to the following:

  • NDIS Quality & Safeguards Commission – Phone 1800 035 544 or TTY on 133677
  • Complete an online complaint form via the NDIS Website

  • Use National Relay Service and ask for 1800 035 544
